This can be a quality improvement initiative based on internal data or a project based on a regulatory change that requires a change in process.

Using an evidence-based solution from research and clinical practice guidelines, create a well-designed project plan that would benefit both patients and the health system.
Your paper should include:

Introduction that includes scope of the problem including internal and external data/statistics to support the problem
Evidence-based solution from research or clinical practice guidelines
Anticipated impact that the project will have and explanation of how findings will close gaps identified by the SWOT analysis and needs assessment
Purpose/Goals
Theoretical nursing models or change theories to support the project
Internal & external stakeholders
Team members including description and roles
Action plan to include implementation strategies, deliverables and technology needs
Time specific milestones
Evaluation metrics for process and outcome indicators
Sustainability measures
Dissemination measures
