Part A

Coral is 28 years old and has mild learning disabilities, a history of mood disorder and psychosis (schizo-affective disorder). She moved to the UK from Turkey as child and English is her second language. She has been known to the Gravesford Borough Council Learning Disability Adults team (‘LD team’) for ten years and receives support services from them including weekly support worker visits to assist with domestic tasks and for general emotional support.

Coral lives in a housing association flat in a supported accommodation service.
Weekly support visits are provided to assist Coral in maintaining her tenancy requirements. She keeps four cats and two dogs in the accommodation. This is against the tenancy rules. She will go for long periods without walking the dogs, and barking is often reported by the neighbours as a disturbance, as are unpleasant smells emanating from the flat into the adjoining communal corridors. Additionally, Coral hoards a variety of items inside the flat including carrier bags, tin cans, newspapers and clothes purchases made at charity shops.

Until recently, Coral has been allowing her boyfriend, Steven, to stay for prolonged periods, the most recent being for approximately 25 weeks. This is also against the tenancy agreement, although he has moved out recently as he can no longer tolerate the mess in the flat.

Sophie Rex contacts the LD team to say that Coral has not been seen leaving home for at least 3 weeks. Will only talk to visitors through letter box. When contact is attempted, she will respond with abusive and threatening language and will not allow anybody inside, other than her mother who visits each day. Sophie states that the flat appears to be packed full of rubbish. In an unhygienic state with cats and dog faeces everywhere. She is concerned that Coral might not be getting food for herself or the animals. Coral appeared dishevelled and neglected when last seen.

Dr Powell wants to arrange a team visit to see how things are as Coral has missed the last 3 appointments with her.
She reports that Coral’s current presentation and disengagement with support is typical of a relapse in her schizo-affective disorder. That it is likely she will need to be detain under the Mental Health Act 1983 in order for her health and social care issues to be address.

Dr Powell is concerned that this current relapse will be the seventh in as many years requiring formal admission under the Act. The most recent being for ten months in 2016.

She is keen to ensure that effective measures are put in place to avoid the situation arising again in the future.

Mrs Asani is also known to hoard items at home. Receives her own support services from the local community mental health team. She does not accept that Coral has a mental disorder. She does accept that Coral’s flat is a problem. But says Coral can stay with her whilst it is sort out and does not need hospital admission. Further, she goes as far as to say she would object to any formal measures being used.
